Session 1: Current Trends in Healthcare
In this Session, we will look at the trends and issues impacting the current healthcare environment and share insights into why the practice of case management is positioned to improve access and quality and contain escalating healthcare costs.
Session 2: History of Case Management
We take a deep dive into the history and background that has shaped the current practice of case management and look at the philosophy and goals case managers in all settings strive to meet. We will also look at the current practice settings that span the continuum of care.
Session 3: CM Competencies and Responsibilities
Case managers may differ according to practice settings, but all have a common role and function that has been documented through certification bodies as well as the Case Management Standards of Practice. In Session Three, we will explore these areas and provide case examples to illustrate each role and function so that learners can better understand the practice.
Session 4: Rehabilitation, Workers Compensation, and Disability Management
An important role of the case manager is to ensure patients receive appropriate care to allow them to reach their full potential. Doing so enables people to be able to work, support their families and maintain their dignity. In this session we will look at processes case managers can use to ensure a timely return to work and learn about the various professionals who are a part return to work process.
Session 5: Data Management
Session Five will review the important role technology is playing in the area of care coordination and how it is changing the delivery of care. We will also look the documentation systems and evidence-based guidelines are decreasing variation of care and improving quality and efficiency within organizations and the system as a whole.
Session 6: Resource Management
Resource Management is a key role for all case managers regardless of setting. In this Session, we will review the various resources and the rules that case managers need to understand in order to ensure the patient has access to the resource they need to meet their individual needs.
Session 7: Utilization Management
Efficient utilization of resource management is key to effective case management. Session seven looks at the history of utilization management, the rules and how case managers can work with the team to ensure patients get the resources medically necessary to meet their needs.
Session 8: Population Health and Disease Management
Understanding a population health and addressing disease management is critical to controlling escalating healthcare costs. This session will review principles and best practices case managers need to incorporate into practice regardless of setting. After completing this unit you will understand:
- What population health and disease management are
- The need for population health and disease management
- The impact disease has on healthcare costs
- Risk stratification and screening tools used in population health and disease management
- The importance of patient engagement in population health and disease management
- Motivational interviewing

Session 9: Healthcare Legislation
Healthcare legislation provides a tool for case managers ensure access to services and protects consumers who have medical, behavioral and social challenges. This session will review key Federal Legislative regulations that case managers need to be aware specifically, how health policies impact CM and reviews specific legislation important to case managers such as The Rehabilitation Act of 1973, The Emergency Medical Treatment and Labor Act (EMTALA), Health Insurance Portability & Accountability Act (HIPAA), Health Information Technology for Economic and Clinical Health Act (HITECH), American with Disability Act, Family Medical Leave Act 1993, Longshore & Harbor WC Act 1927.
Session 10: Reimbursement Systems
Reimbursement issues are a challenge for all case managers in ensuring patients have access to care. Session ten will review the major reimbursement systems so case managers have a working knowledge of each.
Session 11: Legal and Ethical Issues Impacting Case Management
Keeping up to date with legal and ethical issues is an important strategy for all case managers. This session will address key terms and provide examples that each case manager can use to relate to legal and ethical issues impacting the practice of case managers.

Continuing Education credit for this program is awarded by Commonwealth Educational Seminars (CES) for the following professions:
Social Workers: CES, provider #1117, is approved as a Provider for Social Work Continuing Education by the Association of Social Work Boards (ASWB) www.aswb.org, through the Approved Continuing Education (ACE) program. CES maintains responsibility for the program. ASWB Approval Period: 10/5/15 through 10/5/18. Social Workers should contact their regulatory board to determine course approval. Social Workers participating in this course will receive 8.0 clinical continuing education clock hours.
Nurses: As an American Psychological Association approved provider CES programs are accepted by the American Nurses Credentialing Center (ANCC). Every state Board of Nursing accepts ANCC approved programs except California and Iowa, however, CES is also an approved Continuing Education provider by the California Board of Registered Nursing, (Provider Number CEP15567) which is also accepted by the Iowa Board of Nursing. Nurses completing this program receive 8.0 CE hours of credit.
